*For registration under NUS category, at least one of the participants in the team must possess a valid NUS student/staff matriculation number.

 

There must be a mininum of 4 registered players and a maximum of 6 players in each team in order for the team to be successfully registered.

 

Ladies U23 category is open to all females who are born in the year 1993 and after. (i.e. 23 years old as of 2016)

 

For the Mixed Open category, there must be at least 2 females in each team.

 

 

For Netballuxion 2016, Ladies U23 and Ladies Open will be held on Day 1 of the event (including Semi-Finals and Finals). Mixed Open will be held on Day 2.

STEP 1:

Please fill in the registration form: https://mysurvey.nus.edu.sg/EFM/se/54CCD23608F9BE68 (registration ends on 9th July 2016, 2359 hrs).

An acknowledgement email will be sent to the Team Captain (Team Member 1) within 5 days with instructions for Netballuxion 2016.

 

 

STEP 2:

Each participant must fill in both the Indemnity Form and the Health Declaration Form in order to be able to participate in Netballuxion 2016 (where both forms will also be attached in the acknowledgement email). Failure to do so will disqualify the individual as a participant of Netballuxion 2016.

STEP 3:

Submit the individual Indemity Forms and Health Declaration Forms in hardcopy along with the registration fees. Details will be provided in the acknowledgement email.

1. There will be no e-invoice issued. Only an official receipt will be issued after payment is received.
